Supporting Young Adults Through Transition
The Bridge Project is a dedicated initiative by My Planet Health C.I.C, designed to support young adults aged 18–22 as they navigate the transition into adulthood. This stage of life can feel uncertain and overwhelming, and many young people find themselves without the emotional or practical support they need.
Through this project, we provide accessible, meaningful support that helps young adults build confidence, develop essential life skills, and feel more prepared for their future.
Funded to make a difference
We are proud to have received a grant from the National Lottery Community Fund to support and grow The Bridge Project.
This funding allows us to offer free and low-cost services to those who need them most, ensuring that support is accessible, inclusive, and impactful.


Who we support
Phase 1 of The Bridge Project focuses on care-experienced and disadvantaged young adults within the Guildford and Woking communities, who often face additional barriers when accessing mental health support, life skills training, and social opportunities—particularly when statutory services come to an end.
As the project develops, Phase 2 expands support to all young people aged 18–22 in the local area.
What we offer
The Bridge Project provides a combination of counselling and practical workshops designed to support both emotional wellbeing and personal development.
Our services include:

Communication and relationship skills

Practical life skills, including CV writing and financial planning
Each element is designed to equip young people with the tools they need to move forward with confidence.
